Beth began capturing symptoms, questions and concerns in the moment so they would not be forgotten or misrepresented later. Brain fog and variable energy can make it hard to hold the right words in the room. Notes are there to support you, not to speak for you.
FND Forward cannot assess symptoms or replace a qualified professional. Use this page to gather what you want to share, then decide what you are comfortable taking in.

Every section is optional. A few short lines can be enough.

Note what is new, stronger, quieter or happening differently. Describe what you noticed, not what you think it means.
Capture questions while they are still easy to hold. Diagnosis, referrals, work, mobility, communication and safety are common examples. Use whatever matters to you.
Record what you tried, any changes made and what you noticed afterwards. You do not need to decide what caused a change.
How have days, communication, mobility, caring, study or work been affected? What support would help you say that clearly?
Add what has not fitted elsewhere, including something that feels difficult to say aloud. Leave a section blank if it is not useful today.
